Supplier Registration - FFATA Form

This section collects license information related to your organization.  License information relevant to your products/services provided or proposed should be included.  Completion of the information in this section is optional.  However, you must still click on the “complete” at the bottom of the form if you intend to submit this form without any information..

FFATA Form 

Step 1 - If a FFATA form has not yet been completed, you will be prompted to fill out a form collecting license information related to your organization. The form consists of 3 sections. The need to complete the next section will automatically be determined based on your answers to each section. You may ultimately complete 1, 2 or 3 sections. Be sure to fill out all required fields, indicated by a red asterisk (*). 

Step 2 - Once your form has been submitted, you will see a  confirmation that your FFATA has been entered, seen below in red. Then select the green Click to Complete This Section button at the bottom of your screen. You will then see the progress percentage increase appropriately and the module status change to Complete.
